Much of female health care today is related to reproductive
medicine and management of the reproductive cycle. This may be as
routine as the confirmation of a wanted pregnancy, or as specific
as the timing of ovulation in the management of infertility. Later
in life, when the ovaries have stopped producing estrogen, many
women are susceptible to the bone-wasting disease osteoporosis,
with its added risk of hip fracture.
The detection of most female reproductive conditions, whether
pregnancy or ovulation, can be determined by hormone assays. The
measurement of hCG in urine can provide an early diagnosis of
pregnancy and lay the foundation for an optimum program of
antenatal care. In less happy circumstances, early pregnancy
testing is important in those at risk of ectopic pregnancy.
The measurement of luteinizing hormone (LH) in urine has also
been proved as a guide to the monthly "LH surge" in women, and thus
to ovulation, which occurs around 36 hours later. LH testing is of
benefit in the treatment of infertility, where timed intercourse
after ovarian stimulation is an early treatment option.
Sexually transmitted infections (STIs) in women can have serious
effects on their fertility. Successful management of women with
STIs or pelvic inflammatory disease demands a precise
identification of the offending pathogen and is frequently
dependent on the treatment of the infecting partner. Of the
prevalent bacterial STIs, Chlamydia trachomatis is by far
the most common.
Osteoporosis affects one in three women after the menopause and is
causally associated with fracture. However, osteoporosis remains
largely under-diagnosed and under-treated. Bone is metabolically
active throughout life, and it is the imbalance between bone
resorption and bone formation which determines osteoporosis. The
measurement of markers of bone turnover provide a good indication
of osteoporosis risk.
